communication of an event or
announcement. It is not the responsibility of the communication team or the church admin to plan an event upon the receipt of this request.

This year Purpose Church has done a great job ministering to families in need within their neighborhoods and within the church with our vegetable garden. A few of our ministries have teamed up and have made Holiday Meal Baskets available to continue supporting those families in need. If you know a family in the church or a family we have served that needs the gift of a Meal Basket; please fill out this form.
If you desire to use the facilities, then Planning Center Calendar is the place to view the master event schedule, rooms, and resources. You can now view, create, edit, and delete your events on the calendars. The room is not reserved until you have received confirmation from the church office. Funerals and special events take precedence over any scheduled event.

Children’s/Youth Ministry Application
Use this form to report accidents, injuries, medical situations, or behavior incidents. (Incidents involving a crime should be reported directly to the church office.) If possible, the report should be completed within 24 hours of the event.
If you desire to use the facilities please complete this form no more than 6 months in advance of the event or less than 2 weeks prior to the event. Approval will depend on the availability of the dates requested. The room is not reserved until you have received confirmation from the church office. Funerals and special events take precedence over any scheduled event.
If you know of someone (friend or family member) who is either in or going to be in the hospital, nursing home, or is on home-stay, and would like a pastoral contact, please fill out the form below.
This form is for individuals who are requesting benevolence. Please allow 24 to 48hrs before getting contacted.
